Steps to applying to Olivet

Click here to download the application (4 pages).  If you are unable to download it, please contact our Secretary, Chris Bures at chris_davebures@bellsouth.net.  You may also receive an application by submitting your request to: Olivet Private School, 568 SE Port St. Lucie Blvd., Port St. Lucie, FL 34984.  Submit the completed application to the address above, with the $20 non-refundable application fee.

Each new family will participate in a family interview with members of Olivet Private School's Board of Directors.  Upon receipt of your application, you will receive information on our interview process.   

If you do not receive an email confirmation that we are in receipt of your application within 2 weeks of mailing your application, please contact the Secretary via email at chris_davebures@bellsouth.net to make sure it was received.

 


 

OLIVET IS NOT EQUIPPED TO ACCOMMODATE STUDENTS WITH LEARNING DISABILITIES. ALL STUDENTS MUST BE ABLE TO WORK ON AGE-APPROPRIATE GRADE LEVEL AND TAKE THE REQUIRED TESTS.

DOCUMENTATION NEEDED TO JOIN OPS

Statement of Faith, signed as part of the Application Form

Records Request (signed) for last school the students attended, at Interview

Custodial Authorization Form (signed), at Interview

High School Contract (signed by student and parents), at Interview

Tuition (non-refundable), deadline will be given at Interview

Pass a Background Check (sexual predator)

 

Unless transferring from public or private school, you must provide:**

     ~Birth certificate (copy)

     ~HRS Immunization Form or HRS Exemption Form (original)

     ~HRS Health Physical Form (original)

**These may be requested later if the former school does not send correct forms
